“Receive on recording paper”.

How to Subscribe

Send the enclosed “α-ALPHA5 ·αFAX service application form” to DDI Central Office by fax.
FAX number: 0077-2322-9555 (24 hours)
This application form must be completed for use of α-APLHA5 (Not required if application was made at place
of purchase).
